GRANT PORTAL ACCESS REQUEST FORM
Important Information for Contract Grant Writers and Consultants

Only individuals employed by or officially affiliated with the organization may be added as a user to the organization's grant portal account.

Contract grant writers, consultants, or outside vendors who are not staff members should not request direct user access to the organization's account.

Instead, contract grant writers must be invited to collaborate on applications using the portal's Collaborator Tool.
This maintains the organization’s control over its account and ensures accurate communication throughout the grant process.

Submission Requirements:

All fields marked with an asterisk (*) are required.

Requests must include the organization's name and Tax ID number.

Requests must include a brief explanation of why user access is being requested.
Response Time:

Requests will be reviewed within two (2) business days.

Our office is staffed Monday through Wednesday.
Requests submitted Thursday through Sunday will receive a response the following Monday.
Note:
Access is granted at the discretion of Autzen Foundation staff.
Submission of this form does not guarantee access will be approved.
